Job summary

Nutrition that Works, LLC in Florence, SC is seeking a consultant dietitian to join our team for the long-term care community. This role offers a hybrid remote schedule and a flexible 8-16 hours per week.

You will perform clinical nutrition assessments, document using the Nutrition Care Process in the EMR, collaborate with the care plan team, complete the MDS - Section K, calculate TF, and make weight and wound management recommendations.

Qualifications

• Credentialed by the CDR and licensed as a dietitian.
• Strong communication and client-centered approach.
• Experience with medical nutrition therapy and long-term care settings.

Responsibilities

• Complete clinical nutrition assessments.
• Document using Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Provide wound and weight-change recommendations.
