Job summary

The position requires a PharmD or BS in Pharmacy, Indiana license or eligibility, and may require ASHP PGY1/PGY2 residency, board certification, and BLS.

Qualifications

• Requires Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy prior to 2006.
• ASHP-accredited PGY1 or PGY2 residency may be required.
• 0–3 years of relevant experience.
• Current Indiana Pharmacist license or eligibility.

Responsibilities

• Provides comprehensive, value-based medication-related care to improve patient quality of life.
• Collaborates with healthcare professionals to support patient outcomes.
• Ensures safety and effectiveness of the medication-use process and monitors therapy.
