Job summary

Indiana University Health is seeking a pharmacist for a hybrid role supporting primary care in Spencer, IN and virtual offices across the region. The position combines in-person and remote work with flexible day options (5x8h or 4x10h).

The role focuses on medication therapy outcomes, collaboration with care teams, safety and regulatory compliance, and patient confidentiality. Requires PharmD or BS in Pharmacy, Indiana licensure, and ASHP residency if applicable.

Qualifications

• Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
• Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
• Requires 0-3 years of relevant experience.
• Requires current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.
• Board certification or other specialized training may be required.
• Basic Life Support certification through the American Heart Association (AHA) may be required.

Responsibilities

• Provides comprehensive, value-based medication-related care to improve patient quality of life.
• Collaborates with other healthcare professionals to achieve medication therapy outcomes.
• Ensures safety and effectiveness of medication use process and monitors therapy.
• Maintains regulatory compliance and protects patient information confidentiality.
• Provides medication information support to medical and nursing staff and ensures continuity of care.
