Job summary

Altais Medical Group Riverside in Ohio is seeking a CA-licensed Nurse Practitioner with 1+ year of primary care experience to join a collaborative, purpose-driven team. • CA-licensed NP with 1+ year of primary care experience.
• Experience documenting patient information and care plans.
• Ability to work in a collaborative, team-based healthcare environment.

Responsibilities

• Obtain history and perform physical examination
• Complete review of medicines and related instructions
• Review home safety and psychosocial needs
• Fulfill documentation needs for STAR and HCC measures
• Obtain and document all pertinent information in patients' charts and case management notes on the same day as seen
• Instruct patients, caregivers and family to call before accessing medical care unless life-threatening
• Maintain patient safety and confidentiality
• Universal precautions in patient care
• Prescribe/order procedures, treatments and medications within scope of practice
• Develop high level of trust with patients and families
