Qualifications

• CA licensed NP with board certification
• Minimum 1 year of primary care experience

Responsibilities

• Obtain history and perform physical examination
• Complete review of medicines and related instructions
• Review home safety and psychosocial needs
• Fulfill documentation needs for STAR and HCC measures
• Obtain and document all pertinent information in patient charts and case management notes on the day of visit
• Instruct patients, caregivers and family to call prior to accessing medical care
• Maintain patient safety and confidentiality
• Apply universal precautions in all patient care
• Prescribe/order procedures, treatments and medications within scope of practice
• Develop high level of trust with patients and families
