Altais is seeking a Nurse Practitioner in Wisconsin to join our collaborative team serving physicians and patients. You will perform comprehensive health assessments, document care, and support STAR and HCC measures, while maintaining patient safety and confidentiality.

Qualifications

• CA Licensed Nurse Practitioner with 1+ year of primary care experience.
• Strong assessment and documentation skills.
