Job summary

CareOregon seeks a Population Health Program Manager to translate strategy into operational execution for Medicaid and Medicare members. You will lead design, implementation, and evaluation of programs, services, and processes addressing defined populations while advancing health outcomes, equity, quality, cost, and member experience.

Hybrid work, in-office 1-2 days/month. Responsibilities include coordinating initiatives across departments and with external partners, defining success metrics,

Qualifications

• Minimum 6 years of experience in population health, public health, healthcare management, quality improvement, or related field.
• At least 2 years in project or program management.
• Demonstrated matrix leadership (no direct reports required).
• Experience leading cross-functional teams and managing complex initiatives.
• Experience using population health data to drive decisions and action.

Responsibilities

• Lead design, implementation, and ongoing management of population health programs and processes.
• Coordinate initiatives across internal departments and external partners.
• Define success measures and monitor performance with health data and experience metrics.
• Develop strategies to engage members, providers, and community partners.
• Prepare status updates and present recommendations to leadership.
• Ensure programs align with regulatory requirements and best practices.
• Collaborate across departments to align priorities and actions.
• Provide training to internal teams and external partners.
• Monitor regulations and advise on updates to programs and measures.
• Represent organization on local/state/national workgroups.
• Develop and manage project plans with timelines and KPIs.
• Apply quality improvement tools to improve program performance.
