CORA Physical Therapy in Orange Park, FL, is hiring a licensed Physical Therapist as a Digital Health Specialist in a hybrid role—three days in clinic and two days remotely. The DHS delivers virtual care and remote therapeutic monitoring, and helps advance CORA's digital health program.

The role requires a Florida license, a CAPTE-approved PT program, and telehealth experience is preferred. You will collaborate with in-clinic providers and the Director of Digital Health to expandremote services.

Qualifications

• Diploma from a CAPTE-approved Physical Therapy Program.
• Physical Therapy state license (or in process).
• Digital/telehealth experience preferred.

Responsibilities

• Make a powerful impact on your local community through inclusive physical therapy treatment.
• Develop and deliver a personalized plan of care for patients with orthopedic and neurologic diagnoses.
• Objectively measure patient outcomes using cutting-edge software.
• Efficiently document evaluations, treatments, re-evaluations, and discharge notes.
• Use digital platform to deliver telehealth visits including evaluation, assessment, and treatment plan development.
• Perform Telehealth and Remote Therapeutic Monitoring (RTM) services with interactive communication and monitoring.
• Work with operations to deliver digital services and meet patient care and staffing needs.
• Communicate with the primary treating clinician providing revisions in treatment as appropriate.
• Serve as a liaison between providers and Director of Digital Health to optimize remote services.
• Maintain regular reporting and data analysis of regional service metrics.
• Participate in program development and marketing efforts as directed by the Director of Digital Health.
