Job summary

CORA Physical Therapy is hiring a Physical Therapist for a Digital Health Specialist role in Winter Haven. This hybrid position requires 3 days in clinic and 2 days remote, offering telehealth and remote therapeutic monitoring to complement on-site care.

The DHS will develop the digital health program, coordinate between patients and in-clinic providers, and report on service metrics. A Florida PT license is required, with outpatient and telehealth experience preferred.

Qualifications

• Licensed Physical Therapist in Florida.
• Diploma from a CAPTE-approved Physical Therapy Program.
• Experience in outpatient care; telehealth preferred.

Responsibilities

• Deliver physical therapy with personalized plans of care.
• Measure patient outcomes using software.
• Document evaluations, treatments, re-evaluations, and discharge notes.
• Deliver telehealth visits including evaluation, assessment, and plan development.
• Perform RTM services and digital health activities.
• Collaborate with clinicians and Director of Digital Health to grow remote services.
• Analyze regional service metrics and support marketing efforts.
