Qualifications

• Master's degree in social work required.
• Current, unrestricted licensure as an LSW in Ohio required.
• Certification by a nationally recognized care management organization preferred.
• Minimum 2 years of clinical experience in healthcare; pediatric experience preferred.

Responsibilities

• Establish therapeutic, culturally responsive relationships with patients and caregivers to promote engagement.
• Conduct comprehensive biopsychosocial assessments to identify needs, risks, strengths, and barriers to care.
• Identify high-risk patients and prioritize interventions using stratification criteria.
• Develop and maintain family-centered care plans integrating medical, behavioral health, and psychosocial interventions.
• Coordinate care across primary, specialty, hospital, home health, schools, and community agencies.
