Job summary

Acentra Health is seeking a part-time PASRR Clinical Evaluator to join our team. This role handles PASRR evaluations, level of care recommendations, and ensures care quality aligns with regulations.

The position is a hybrid mix of telehealth and face-to-face assessments in nursing facilities and hospitals around Yakima, Washington. 3+ years in assessment and licensure in WA are required; IDD/SMI experience is preferred.

Qualifications

• 3+ years of experience in assessment and/or diagnosis of mental/psychiatric disorders, including complex differential diagnoses.
• 1+ years of experience with individuals with intellectual/developmental disabilities or serious mental illness.

Responsibilities

• Perform PASRR Level II assessments by reviewing records and gathering information from collateral sources.
• Develop evidence-based level-of-care recommendations and determine appropriate next steps for long-term care placement.
• Review medical records and provide clinical recommendations in line with regulatory requirements and turnaround times.
• Communicate with healthcare professionals, patients, families, and internal partners to support assessment processes.
• Utilize technology and software platforms to manage assessments and track activities.
• Ensure compliance with workflows and complete assessments within scope of practice and contractual requirements.
• Determine a clinical diagnosis accounting for psychological and behavioral factors.
• Assess psychological, emotional, behavioral, and physical functioning.
• Adhere to HIPAA and related Privacy and Security Rules.
