Job summary

Qualifications

• Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program.
• Registered Dietitian Nutritionist (RD/RDN) with current credentialing.
• Current Pennsylvania state licensure/certification as required.
• Minimum 1 year clinical experience preferred.
• Skilled nursing or long-term care experience preferred.
• Excellent communication skills with residents, families, and healthcare teams.
• Ability to apply critical thinking to develop, implement, and monitor care plans.

Responsibilities

• Conduct comprehensive nutritional assessments on new admissions and quarterly thereafter, with more frequent reviews for high-risk residents.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition education and counseling as needed.
• Collaborate with physicians, nurses, therapists, and other care team members.
• Ensure compliance with state and federal regulations (CMS DOH) and maintain documentation.
• Monitor resident weight changes weekly and/or monthly.
• Participate in quality improvement initiatives to enhance nutrition care and outcomes.
• Conduct monthly kitchen sanitation audits and document findings.
