AccentCare is seeking a Director of Palliative Care in West Allis, WI to oversee inpatient hospice services across settings. The role emphasizes physician supervision, service coordination, and delivery of high‑quality patient care in collaboration with the Medical Director and interdisciplinary teams.

The incumbent will lead education, participate in leadership meetings, and drive best practices in hospice care while coordinating patient admissions and transitions.

Formación

• Licensed MD or DO with experience in caring for chronically or terminally ill patients.
• Active DEA license.
• Strong knowledge of primary medical care principles.

Responsabilidades

• Provide ongoing education to inpatient staff in coordination with clinical leadership.
• Lead monthly leadership meetings with Medical Director and inpatient leadership.
• Support and advise physicians and patient care teams to ensure high‑quality care.
• Collaborate with attending physicians on prognosis and care planning.
• Oversee admissions, discharges, and transitions within inpatient settings.
• Ensure continuity of care and effective handoffs between care teams.
• Promote best practices in hospice and palliative care across inpatient settings.
• Conduct periodic reviews of clinical care quality delivered by inpatient physicians.
• Perform other duties as assigned by Medical Director/Executive Director.
• Occasionally visit hospitals for meetings and leadership needs.
