Job summary

Humana is seeking an Onsite Care Coordination Nurse in Greenwood, SC to support members during critical care moments. You will perform face-to-face assessments, coordinate discharge planning, and collaborate with providers to ensure smooth transitions from hospital to home.

The role supports a hybrid work model with some in-person facility engagement. Required: SC RN license, 3+ years acute care, discharge planning experience, and proficiency with Microsoft Office and EMRs.

Qualifications

• Current, unrestricted RN license in South Carolina.
• Minimum 3 years acute care nursing experience (medical-surgical, critical care, etc.).
• Experience with discharge planning or care coordination in hospital or health plan.
• Able to complete documentation accurately and promptly.

Responsibilities

• Conduct face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning and post-discharge follow-up with PCPs.
• Identify gaps in care and develop strategies with care teams.
• Collaborate with providers and facility case managers for coordinated care.
• Educate on diagnoses, medications, and preventive measures.
• Maintain infection control and patient privacy standards.
• Complete all required documentation in a timely manner.
