The role blends on-site visits in Huntingdon, TN with remote work from home, supported by a dedicated scribe team for efficient documentation.

Mansioni

• Provide advanced nursing care to residents in skilled nursing facilities, focusing on chronic disease management and prevention.
• Conduct patient assessments, develop care plans, and provide medication management and education.
• Collaborate with facility staff, families, and other healthcare professionals to ensure comprehensive care.
• Utilize RPM, CCM, and CoCM to proactively manage patient care.
