Job summary

Mindful Care is hiring for care managers to conduct thorough assessments, coordinate with patients and providers, and support care management and documentation. The role emphasizes collaboration, timely communication, and navigating insurance and records to ensure smooth patient care.

The position requires 3+ years in administrative or case management roles, an associate degree in social work or psychology, and proficiency with Google Suite and MS Office.

Qualifications

• 3+ years of experience in administrative functions or case management.
• BLS or CPR/First Aid certification preferred.
• Experience with Transcranial Magnetic Stimulation (TMS) administration preferred.
• Proficiency with Google Suite and Microsoft Office (Word, Excel, Outlook, Teams).
• Associate degree in social work or psychology required; bachelor's degree preferred (experience may substitute).
• Excellent verbal and written communication skills required.
• Reliable transportation and home internet access required.

Responsibilities

• Conduct thorough assessments of patient concerns and expectations.
• Maintain effective communication with patients, pharmacies, referral sources, hospitals, and therapists.
• Support patients in obtaining medical records and supporting forms; manage prescriptions and referrals.
• Assist with TMS operations when applicable; secure prior authorizations as needed.
• Document clinical notes and ensure timely submission for review and signature (72 hours).
