Job summary

VNA Care in Worcester, MA is seeking an LPN Clinical Admissions Coordinator to join the team. The role requires reviewing clinical information prior to patient admission to ensure a smooth pathway and an exceptional patient experience.

You will coordinate with branches and internal teams while maintaining professional relationships with customers. On-site Monday and Wednesday 8:00AM-4:30PM; remote Tue, Thu, Fri 8:00AM-4:30PM; weekend/holiday rotation.

Qualifications

• Minimum 2 years related experience including medical/surgical and community health and/or experience with a managed care organization.
• Current LPN license required.
• Previous intake, liaison or discharge planning experience is preferred.
• Knowledge of home care standards and practices required.
• Knowledge of medical terminology and treatment modalities of patient diagnoses required.

Responsibilities

• Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient.
• Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers.
