Job summary

PruittHealth is expanding its NP program in a hybrid role with limited daytime travel to corporate offices and SNFs. The Lead Nurse Practitioner will develop protocols, recruit and train NPs, and work with physicians and care teams to improve the PHP model of care.

The role requires strong leadership, clinical oversight, and experience with geriatric populations, CMS/HEDIS documentation, and data-driven care improvements.

Qualifications

• Graduate of an accredited Adult, Geriatric or Family Nurse Practitioner Master’s Degree program.
• Current and unrestricted RN and NP licensure in state of practice.
• Current CPR certification.

Responsibilities

• Provides expertise to training and onboarding of Plan Provider NPs.
• Directs Quality Assurance/Performance Improvement activities.
• Articulates plan benefits and policies to providers, members and decision makers.
• Coordinates care across levels of care and communicates ICP within 24 hours.
• Leads NP plan providers to develop Care Plans with measurable outcomes.
• Collaborates with SNF DONs, Administrators, ISNP VP Operations and Medical Director.
• Interprets data and reports with EHR tools to support CMS/HEDIS metrics.
