Qualifications

• MSW from CSWE-accredited program required.
• Current Texas LCSW licensure required.
• Preferred: 3 years post-degree clinical experience in healthcare.
• Preferred: Certified Clinical Transplant Social Worker (CCTSW) credential and transplant experience.
• Bilingual in English and Spanish; BLS certification is a plus.

Responsibilities

• Perform biopsychosocial assessments and document interactions in the medical record.
• Provide clinical interventions and develop individualized treatment plans with patients and families.
• Coordinate care with interdisciplinary teams and assist with housing, transportation, and immigration supports; refer to disability services and insurance resources.
• Participate in transplant team conferences, policy development, and quality improvement initiatives.
• Engage in research, education, and interdisciplinary activities to improve patient care.
