Job summary

Sentara Health Plans is seeking a Licensed Clinical Social Worker (LCSW) to join the outpatient care team in Williamsburg, VA. The role offers a hybrid work model with initial training, Monday–Friday schedule, and a focus on psychosocial assessment and integrated patient planning.

The LCSW will provide individual/family counseling, crisis intervention, and referrals within a multidisciplinary team to support patients and families in achieving optimal care outcomes.

Qualifications

• Master's degree in social work required.
• Licensed Clinical Social Worker (LCSW) required.
• Minimum of 2 years of social work experience.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide individual, family and group counseling as needed.
• Collaborate with a multidisciplinary team to plan care.
• Deliver crisis intervention and referrals to resources.
• Develop and adjust patient care plans based on progress.
