Qualifications

• Master's degree in Social Work required.
• Licensed Clinical Social Worker (LCSW) required.
• Minimum of 2 years of social work experience.

Responsibilities

• Provide clinical intervention and crisis intervention services as part of an integrated care team.
• Conduct psychosocial assessments and develop patient/family care plans.
• Refer patients to community resources and coordinate with other professionals.
• Deliver individual, family and group counseling and education as needed.
• Participate in rounds and monitor plan progress to adjust care.
