Qualifications

• Bachelor’s or Master’s in Social Work, Psychology, Public Health, or related field; 5+ years in case management and behavioral health with leading teams.
• Solid Medi-Cal knowledge, care coordination, and regulatory compliance; CalAIM ECM, MCP workflows, billing, and documentation familiarity.
• Clinical licensure (LCSW) required; experience with Managed Care Plans and program development preferred.
• Proficiency in EHR and case management software, data tracking, and Microsoft Office; strong analytical and executive-level communication skills.

Responsibilities

• Lead, supervise, and performance-manage ECM Care Managers, establishing KPIs aligned with CalAIM metrics.
• Ensure regulatory compliance with DHCS, Medi-Cal, MCP contracts, HIPAA, and audits through documentation and QA oversight.
• Provide clinical leadership on high-acuity cases, oversee biopsychosocial assessments, risk stratification, crisis intervention, and care plan approval.
• Build cross-functional partnerships to improve coordination and reduce ER utilization.
• Support program development and BH integration by optimizing workflows and contributing to growth initiatives.
