Qualifikationen

• Master’s degree in Social Work (MSW) and Illinois licensure or eligibility.
• Minimum 2–5 years of clinical social work experience in healthcare or outpatient settings.
• Experience with crisis intervention and trauma-informed care.
• Proficient in maintaining confidential, accurate clinical documentation.

Aufgaben

• Conduct comprehensive clinical assessments and diagnostic evaluations to identify patients’ behavioral health, emotional, psychological, and psychosocial needs.
• Provide goal-focused, evidence-based, time-limited psychotherapy (individual, couples, family, group).
• Develop individualized treatment and care plans with measurable goals.
• Monitor, evaluate, and document patient progress toward treatment goals and adjust interventions as needed.
• Provide crisis intervention and stabilization services and refer to higher levels of care when necessary.
• Coordinate care with psychiatrists, physicians, nursing staff, case managers, and other team members.
• Maintain timely electronic health records, assessments, treatment plans, and progress notes.
• Maintain confidentiality and exercise sound clinical judgment per HIPAA and ethics.
• Provide clinical supervision and mentorship to trainees or learners as appropriate.
