Job summary

BioPlus Specialty Pharmacy, part of the Elevance Health family, seeks a Paragon Intake Clinician to ensure accurate intake information and patient demographics entry. The role supports setting up home services for new referrals and collaborates with clinical and reimbursement teams.

The position requires in-office presence 1-2 days per week with hybrid/virtual flexibility and occasional travel to worksites. LPN/LVN or RN license and infusion nursing experience are required.

Qualifications

• Requires HS diploma or equivalent and 2 years nursing infusion therapy experience.
• Requires a current, active, valid and unrestricted LPN/LVN, or RN license to practice as healthcare professional within scope of license in applicable state(s) or territory of the United States.

Responsibilities

• Obtain and document patient referral data promptly.
• Enter patient demographics into the computer system accurately.
• Notify pharmacy and reimbursement managers of pending referrals.
• Forward all patient documentation to clinical staff in appropriate location and to reimbursement staff.
• Communicate with patient/caregiver all applicable information regarding therapy and financial arrangements.
• Assist in effective communication between the patient and the company, including documentation of suggestions, complaints, and requests.
• May require extensive travel to worksite and other locations as necessary.
