The APC will ensure the delivery of high-quality clinical and home-based patient care, while promoting optimal health outcomes and providing cost-effective solutions that adhere to Alignment Health’s policies, as well as state and federal regulations.

You’ll play a vital role in assessing, developing, and coordinating care options tailored to the needs of our members, who are typically 65 and older. With home and virtual visits ranging from 45 to 60 minutes, you’ll have the opportunity to provide focused, compassionate care, whether it’s an initial visit, an annual check-up, or when additional support is needed to address medical and social challenges.

Requirements:

Experience:

1. Preferred: One (1) year of prior clinical or home care experience. Previous EMR experience preferred. Experience in care of older adult (geriatric) patients preferred.

Education:

1. Master’s degree from an accredited NP Program or PA program.

Licensure:

1. Required: Active California state Nurse Practitioner or Physician Assistant license.
2. Active RN License and Furnishing number (Must, upon hire) NP only.
3. Active Nurse Practitioner Board Certification/Physician Assistant Certification.
4. NPI Number, DEA, Valid BLS.
5. Valid California driver license and current automobile insurance.

Pay Range: $113,332.00 - $169,999.00
