Job summary

Pinellas County Human Services is seeking an experienced Human Services Eligibility Case Manager (Case Manager 1) for a hybrid role in St. Petersburg, FL.

You will screen applicants, determine eligibility for medical assistance, provide referrals, and manage a caseload with data entry and documentation in county databases. You will collaborate with community partners, attend trainings, and support residents with access to housing, healthcare, and financial resources.

Qualifications

• Experience in social/human services case management or related field.
• Degree in Social Work, Psychology, Sociology, or a related field.
• Must have valid Florida Driver License and ability to travel to offsite locations.

Responsibilities

• Screen and interview applicants in person and by phone to determine eligibility for benefits.
• Process applications to determine medical assistance eligibility and refer ineligible applicants to resources.
• Provide information and referral services to clients; work with partners to access services.
• Maintain active caseloads and enter notes and documents in databases.
• Attend meetings/trainings and perform related duties as assigned.
