Job summary

Qualifications

• Bachelor's degree in Physical Therapy required or Master's/Doctorate preferred.
• Experience in lieu of a degree not accepted.
• State PT license and BLS certification required/preferred.

Responsibilities

• Establishes therapy goals and formulates a treatment plan based on evaluation results.
• Supervises PTAs according to policy and state regulations.
• Provides individual, family and group therapy sessions for outpatient clients.
• Participates in hospice group and home health case conferences.
• Contributes to discharge planning.
• Performs initial clinical assessments and documents in EMR.
• Maintains accurate patient data in medical records.
