Job summary

St. Luke's University Health Network in Bethlehem, PA seeks a Clinical Health Coach for a hybrid role to promote healthy behaviors among employees and their families. You will collect data, analyze needs and design education materials to support program success.

The ideal candidate has an RN or RD license (MPH with coaching credential) and at least 2 years of health coaching or health education experience; bilingual Spanish is preferred and motivational interviewing experience is a plus.

Qualifications

• Clinical licenses such as RN or RD; MPH with coaching credential or CHES preferred.
• At least 2 years health coaching or health education experience.
• Experience with employee populations preferred; motivational interviewing is a plus.
• Bilingual Spanish preferred.

Responsibilities

• Serves as a health coach for a designated group of employees/spouses; initiates contact and follows through on care navigation.
• Maintains timely, effective communication with employees/spouses and education participants.
• Provides individual or group counseling and education to patients as needed.
• Identifies barriers, develops treatment plans, and supports health education program participation.
• Engages with a diverse population and maintains proper documentation and data reporting.
• Supports customers and teams by designing, implementing and evaluating health education materials.
