Job summary

Bluestone in Minnesota is seeking a Care Coordinator to empower seniors to navigate their healthcare journey and live healthier, more independent lives. You will manage DHS and CMS requirements for MSHO/MSC+ members, and perform home visits to assist with access to medical care, waiver services, and community resources.

This is a hybrid role based in Winona County, with travel to members' homes and assisted living facilities, balancing 50–70% local visits and 30–50% remote work from home.

Qualifications

• Current MN license as Social Worker (LSW/LMSW/LISW/LICSW) or MN RN license.
• Experience with geriatric or disabled populations in case management / care coordination preferred.
• Valid driver’s license is required.

Responsibilities

• Coordinating face-to-face visits
• Managing the Elderly Waiver
• Conducting annual PCA assessments and living tools for residents in Assisted Living
• Reviewing health needs, setting goals, and creating individualized care plans
• Connecting members with community and state resources/services
• Helping members remain safely in their homes and communities
• Completing required documentation
• Collaborating with medical care teams to meet quality measures
• Supporting transitions of care and ensuring safe discharge and follow-up
