JARVIE PROGRAM LLC seeks a compassionate Social Worker / Geriatric Care Manager to help seniors live with dignity and independence. You will conduct in-home visits, coordinate benefits, and advocate within healthcare and social services, combining field work with administrative duties from Queens, NY.

Ideal candidates hold an LMSW or LCSW, have experience with older adults, and are adept at documentation, scheduling, and multidisciplinary collaboration to improve client outcomes.

Qualifications

• LMSW or LCSW license is required.
• Experience with older adults, care management, or related human services fields.
• Knowledge of Medicare, Medicaid, Social Security, senior housing, and community-based services.
• Excellent interpersonal, communication, and advocacy skills.
• Strong organizational, documentation, and time-management abilities.
• Ability to work independently while collaborating with a team.
• Proficiency with Microsoft Office and electronic documentation systems.

Responsibilities

• Provide comprehensive care management and ongoing support to older adults living in the community.
• Conduct regular in-home visits and assessments in your assigned territory.
• Advocate for clients and help them navigate healthcare, housing, financial, and social service systems.
• Assist clients in accessing benefits, entitlements, and community resources.
• Develop individualized service plans and monitor client progress.
• Collaborate with family members, healthcare providers, and other members of the client’s support network.
• Maintain timely and accurate case documentation.
• Participate in weekly team meetings and case conferences.
• Contribute to program development and continuous quality improvement initiatives.
