KVC West Virginia is seeking a Foster-Adoptive Care Case Manager in Charleston, WV. This full-time hybrid role focuses on supporting foster and adoptive families through visits, advocacy, and guidance, with two home visits per month and monthly school visits.

The position requires a Bachelor's degree in a social service-related field, at least two years in children and family services, and a valid driver's license.

Formación

• Bachelor's Degree in a social service field.
• Licensed by WV Board of Social Work Examiners preferred but not required.
• Minimum of two years of experience in children and family services.
• Valid driver's license and auto insurance required.
• Knowledge of WV Medicaid Program and foster care system preferred.

Responsabilidades

• Provide visits, advocacy, and guidance to foster and adoptive families.
• Conduct home visits at least twice monthly and school visits monthly.
• Attend MDT and IEP meetings to advocate for children.
• Collaborate within a multidisciplinary treatment team.
• Travel throughout the service area as required.
