Job summary

Riverside School Age East is seeking a Consumer Services Coordinator to coordinate case management services for individuals with developmental disabilities in the Riverside East area. The role emphasizes person-centered planning, IPP development, and progress reporting, with emphasis on interagency collaboration.

You will advocate for consumers and families, attend relevant IEP/ITP meetings, and ensure timely documentation and compliance.

Qualifications

• Bachelor’s degree from an accredited college or university and one year of experience in case management in developmental disabilities, social work or related field.
• OR Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or related field.

Responsibilities

• Coordinate CSC daily responsibilities in providing case coordination services.
• Use person-centered planning to create and execute IPP, generate progress reports and conduct annual reviews.
• Evaluate progress and implement IPP addendums as needed.
• Provide advocacy for consumers and families; attend IEP/ITP/IHC meetings and prepare paperwork.
• Coordinate medical, psychological, educational and other consumer assessments.
• Ensure Medicaid Waiver standards are maintained and documentation is timely.
• Arrange placement in residential facilities, day programs or other training as clinically indicated.
• Complete required documentation, audits and reports and maintain consumer files.

Education

Bachelor’s degree from an accredited college or university
