Job summary

Pharma-Care in New Jersey seeks a Consultant Pharmacist for a hybrid Long Term Care role. The position is 24–30 hours weekly and requires a NJ Board of Pharmacy license. Expect regional travel within Atlantic county and a clinical focus on optimizing medication regimens.

You will collaborate with physicians, develop protocols, and lead antimicrobial stewardship efforts while educating nursing staff and supporting formulary initiatives.

Qualifications

• Licensed pharmacist with NJ Board of Pharmacy license.
• Experience in long-term care preferred.
• Willingness to travel regionally.
• Ability to work in a hybrid schedule.

Responsibilities

• Travel to long term healthcare facilities within the region.
• Evaluate the appropriateness and effectiveness of medication regimens.
• Collaborate with physicians to manage therapy and ensure patient needs.
• Develop and implement protocols and antimicrobial stewardship initiatives.
• Educate nursing staff and participate in QAPI meetings and drug safety initiatives.
