RD Nutrition Consultants LLC is seeking a Clinical Registered Dietitian for a part-time, hybrid role in Carlisle, PA. The position offers 8-12 hours per week per facility, with potential 16-24 hours to cover both sites, and a flexible schedule combining on-site and remote work.

Responsibilities include comprehensive nutritional assessments, care plan development, nutrition education, and collaboration with the care team, CMS/DOH compliance, and menu reviews.

Formación

• Bachelor’s or Master’s degree in Dietetics, Nutrition, or a related field from an accredited program.
• Registered by the Commission on Dietetic Registration.
• Current Pennsylvania state licensure/certification as required.
• Minimum of 1 year clinical experience preferred.
• Skilled nursing or long-term care experience preferred.
• Excellent communication skills for effective interaction with residents, families, and healthcare teams.
• Ability to apply critical thinking to assess objectives and develop plans.
• Proficiency in providing general nutrition interventions.
• Familiarity with state survey processes and nutrition care regulations.
• Competency in electronic charting systems.

Responsabilidades

• Conduct comprehensive nutritional assessments on new admissions and quarterly thereafter, with more frequent reviews for high-risk residents.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition education and counseling as needed.
• Collaborate effectively with physicians, nurses, therapists, and other members of the interdisciplinary care team.
• Ensure compliance with CMS and DOH regulations.
• Monitor residents’ weight weekly and/or monthly.
• Participate in quality improvement initiatives to enhance nutrition care.
• Conduct monthly kitchen sanitation audits and document findings and corrective actions.
• Review and approve menus on a semi-annual basis.
• Perform other tasks as assigned within scope of practice.
