Job summary

RareMed is seeking a Case Manager in the Coraopolis, PA area to serve as the primary contact for the patient journey. You will coordinate services with internal operations and program management, assess patient needs, and ensure timely access to therapies while navigating payer processes and prior authorizations.

The role requires strong communication and analytical skills, with 2+ years in healthcare and familiarity with pharmacy or insurance settings.

Qualifications

• Bachelor’s Degree or 3–5 years case management or patient advocacy experience.
• 2+ years experience in healthcare setting, pharmacy, or insurance background.
• Strong analytical and organizational skills with attention to detail.
• Excellent verbal and written communication skills.

Responsibilities

• Primary point of contact for patient journey.
• Coordinate services with internal operations and program management.
• Assess, plan, implement, and evaluate patient needs throughout journey.
• Understand disease states, products, and support programs.
• Work independently to complete enrollment, answer inquiries, and coordinate access to therapies.
• Process referrals to ensure timely therapy access.
• Collaborate with internal/external teams to expedite referrals and provide reimbursement support.
• Maintain frequent contact to resolve inquiries with patients and staff.
• Provide exceptional customer service and escalate where needed.
• Support payer processes and prior authorization navigation.
