Sanitas in the Houston area is seeking a Behavioral Health Counselor to join our primary care team and deliver comprehensive mental health services via telehealth and in-person visits. You will develop care plans, document thoroughly, and collaborate with primary care providers and specialists.

The ideal candidate holds a Master’s degree in counseling or related field, has at least one year of relevant experience, and is licensed or eligible for BLS certification.

Qualifikationen

• Master’s degree in counseling or related field from an accredited program.
• 1 year of relevant experience.
• BLS certification.
• Able to establish and maintain effective working relationships with mentally or emotionally disabled persons and their families.
• Flexible schedule Weekdays – weekends.
• Good communication and listening skills.
• Ability to show empathy and compassion.

Aufgaben

• Develop and implement care plans.
• Complete and maintain all documentation in a timely and accurate manner according to federal, state and center guidelines.
• Collaborate with health professionals including primary care providers, care nurses, care coordinators, social workers and psychiatrists.
• Ensure continuity of care.
• Provide individual, family or group educational interventions.
• Guide patients in developing skills and strategies for dealing with their diagnosis.
• Assist patients in adjusting to life and making changes.
• Participate in weekly caseload consultation and complex case review meetings.
• Facilitate care plan adjustments with medical providers.
• Facilitate referrals to appropriate providers or in-house programs as clinically indicated.
• Connect patients and families with community resources.
• Guide clinical staff on how to manage common behavioral health situations.
• Screen and assess patients for mental health and substance abuse disorders.
• Support psychotropic medication management as prescribed by medical providers.
• Track treatment response and monitor changes in symptoms and side effects.
• Provide or facilitate referrals to psychosocial interventions.
