CommUnityCare in Bastrop, Texas is looking for a Behavioral Health Consultant (BHC) to join their multidisciplinary team in a fast-paced medical clinic. The BHC will engage in integrated care, document patient care plans, and promote health initiatives tailored for diverse patient needs.

A Master’s degree in a relevant field and a current Texas licensure as LCSW or LPC are required. Preferred candidates have 2 years of clinical experience in mental health, ideally in a primary care setting. Join us in serving our community effectively.

Qualifications

• At least 2 years of clinical experience in mental health.
• Current Licensure as LCSW or LPC in the State of Texas.
• Experience with population-based practice management.

Responsabilités

• Collaborate on a multidisciplinary team to provide integrated care.
• Engage in behavioral health model to promote patient health.
• Document patient care plans in electronic health records.
• Monitor site’s behavioral health program and make recommendations for improvement.
