Vynca Inc. in Los Angeles County is seeking a Behavioral Health Case Manager to join the Enhanced Care Management team.

The role blends in-person and remote care management, coordinating with physicians, social services, and families to ensure aligned needs and timely access to services. Applicants must hold a California license (ACSW/LCSW/LMFT/LPCC), have 1+ year of experience, and be willing to travel within the Los Angeles area; Monday–Friday 8:30–5 with some evenings and weekends.

Qualifications

• Active, unrestricted license in California (ACSW/LCSW/LMFT/LPCC) is required.
• 1+ year of experience as a care manager, care navigator, or community health worker; 2+ years preferred.
• Willing to work Monday-Friday 8:30am-5:00pm, with flexibility for evenings and weekends.
• Knowledge of government/community resources related to social determinants of health is preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services.
• Oversee development of client care plans and SMART goals.
• Provide services where the member resides or accesses care (office, telehealth, or field-based).
• Connect clients to social services and supports as needed.
• Advocate with healthcare professionals (e.g., PCP).
• Utilize evidence-based practices (Motivational Interviewing, Harm Reduction, Trauma-Informed Care).
• Document progress in the CRM/EMR systems and complete monthly reporting.
