Job summary

Home Physicians Group is seeking an experienced Family Practice or Internal Medicine APRN/APRN or PA to join a hybrid remote and in-home care team. The provider will deliver virtual visits for homebound patients and in-home visits 1-2 times per week.

The position emphasizes high-quality, patient-centered care within geriatric populations served across Central Florida, with competitive compensation and bonuses.

Qualifications

• Licensed APRN or PA with Florida licensure.
• Current Florida DEA license for APRN.
• Proven APRN/PA experience in clinical settings.
• Professional bedside manner and patient focus.
• Excellent oral and written communication skills.
• Bilingual preferred; Spanish or other language helpful.

Responsibilities

• Assess, diagnose, and treat assigned patients.
• Administer nursing care to ill, injured, or disabled patients.
• Diagnose and establish patient treatment plans.
• Monitor and report changes in patient symptoms or behavior.
• Communicate with collaborating physicians or specialists regarding patient care.
• Educate patients about health maintenance and disease prevention.
• Facilitate referrals to other healthcare professionals and facilities.
• Maintain accurate patient medical records.
• Provide advice and emotional support to patients and families.

Requirements

• Duly licensed provider in the State of Florida, with unrestricted licenses to practice
• Current DEA license for APRN
• Proven experience as a APRN
• Professional bedside manner
• In-depth knowledge of legal medical guidelines and medicine best practices
• Excellent oral and written communication skills
• Bilingual preferred
• 1+ years as APRN or PA experience
• Responsible for the assessment, diagnosis, and treatment of assigned patients
• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
