Job summary

Home Physicians Group seeks an experienced ARNP/APRN or Physician Assistant to join its hybrid remote and in-home care team in Florida. The provider will deliver virtual visits for homebound patients and conduct in-home visits 1–2 times per week, focusing on geriatric care and continuity of care.

Qualifications

• Duly licensed provider in the State of Florida with unrestricted licenses to practice.
• Current DEA license for APRN/PA
• Proven experience as APRN or PA
• Excellent oral and written communication skills
• Professional bedside manner
• In-depth knowledge of medical guidelines and best practices
• Ability to build rapport with patients and facilities
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment

Responsibilities

• Provide virtual visits for homebound patients.

The ideal candidate will provide high-quality, patient-centered care through virtual visits for homebound patients, along with in-home visits 1-2 times per week.

Requirements :

• 1+ years as APRN or PA experience
• Responsible for the assessment, diagnosis, and treatment of assigned patients
• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
