Job summary

Home Physicians Group, a Central Florida practice, seeks an experienced ARNP/APRN or Physician Assistant to join a hybrid remote and in-home care team providing virtual visits and in-home visits 1-2 times per week for homebound geriatric patients.

You will collaborate with facilities, families, and care teams to improve outcomes, maintain continuity of care, and deliver high-quality patient care while enjoying flexible scheduling and a supportive team environment.

Qualifications

• Must hold an unencumbered Florida medical license as APRN/PA.
• Current DEA license for APRN or physician assistant.
• Proven experience in APRN or PA roles, with geriatrics preferred.

Responsibilities

• Provide assessment, diagnosis and treatment for homebound patients.
• Deliver in-home care 1–2 times per week and virtual visits.
• Collaborate with physicians, facilities and care teams to improve outcomes.

Requirements :

• Duly licensed provider in the State of Florida, with unrestricted licenses to practice
• Current DEA license for APRN
• Proven experience as a APRN
• Professional bedside manner
• In-depth knowledge of legal medical guidelines and medicine best practices
• Excellent oral and written communication skills
• 1+ years as APRN or PA experience
• Responsible for the assessment, diagnosis, and treatment of assigned patients
• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members
• Familiarity with medical software and equipment
• Ability to build rapport with patients and facilities
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
