UC Davis Health is seeking a Human Services Supervisor to lead the Home Care Services Department across Hospice, HBPC, and Home Health. The role includes clinical supervision, staff development, and collaboration with interdisciplinary teams to ensure high-quality patient-centered care.

The Human Services Supervisor is a licensed clinical social worker (LCSW) who provides clinical, operational, and supervisory leadership for Human Services within the UCDH Home Care Services Department, with responsibility for Hospice, including Volunteer and Bereavement programs, Home-Based Palliative Care (HBPC), and Home Health. The position also supports Medical Aid in Dying (MAID) Navigators through collaboration and consultation.
The supervisor plans, coordinates, supervises, and evaluates Human Services activities; oversees staff assignments, workload, productivity, orientation, training, and performance; and provides clinical consultation and support for complex patient, family, and caregiver needs. The position promotes interdisciplinary collaboration, regulatory compliance, quality improvement, policy implementation, and program development.
The supervisor participates consultatively in interdisciplinary team meetings, Hospice IDG activities, family meetings, case conferences, and department initiatives. The position also maintains responsibility for professional LCSW practice and provides direct clinical services as needed, including psychosocial assessment, counseling, crisis intervention, resource coordination, advocacy, and end-of-life support.
The position works independently under the direction of the department manager, exercises sound clinical and organizational judgment, and maintains professional, clinical, and leadership competencies.

Human Services supports the psychosocial and spiritual well-being of patients and families and promotes quality of life, continuity of care, and patient- and family/caregiver-centered services.
Human Services includes licensed clinical social workers (LCSWs), master's-level social workers (MSWs), chaplains, as well as both a volunteer and bereavement program coordinator. Services include psychosocial and spiritual assessment, counseling, crisis intervention, care coordination, resource linkage, advocacy, discharge planning, and support related to serious illness, end of life, grief, and loss.
Services are delivered through interdisciplinary collaboration with physicians, nurses, and other health care professionals, with a focus on compassionate, equitable, culturally responsive, and high-quality care.
Home Care Services is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Home-Based Palliative Care. This position is part of the operations leadership team and supervises various human services staff within the home hospice, home-based palliative care, and home health programs.
