HealthierHere is Seattle-based, a nonprofit collaborative advancing health equity across King County. The HealthierHere Community Hub centralizes referrals and coordinates health-related social needs services with a focus on home modifications and accessibility.

The HRSN Eligibility and Referral Specialist- Home Modifications will support eligibility, assessments, enrollments, and vendor bidding, reporting to the Director of Program Operations.

Qualifications

• 2+ years of combined education and/or professional experience in health, human services, social services or related fields.
• Strong customer service skills and ability to connect authentically with diverse community members.
• Excellent written and oral communication skills, attention to detail, organization, and coordination.

Responsibilities

• Review Medicaid status and other criteria to determine client eligibility for Medicaid waiver funded HRSN services.
• Enter eligibility data into the Connect2 Coordinator system.
• Respond to referrals for HRSN services from Community Hub CBWs and contracted providers.
• Pre-screen potential HRSN recipients using the HRSN decision tool to determine likely eligibility.
• Complete HRSN assessments with eligible clients over the phone and document in GetCare system.
• Enroll eligible clients in HRSN services and complete service authorizations in GetCare system.
• Manage the bidding process for Home Accessibility Modifications and Remediation services.

TheHealthierHereCommunity Hub is a community-centered entity that centralizes referrals from across King County to connect people to health-related social needs services and benefits. The Hub brings together community-based, behavioral health, and healthcare organizations to provide culturally responsive care coordination support for community members. The Hub’s benefits and services are free for everyone.

TheHRSNEligibility andReferral Specialist- HomeModificationswill initiallyreport directly to theDirector of Program Operations andis responsible forsupporting the HRSN Screening and Referral contract thatHealthierHerehas with the City of Seattle Aging & Disability Services.

TheHRSN Eligibility and Referral Specialistwillconfirm client eligibility forcontracted Health Related Social Needs (HRSN)services,performHRSN assessments with clients over the phone,enter data intwo client Management systems(GetCareand Connect2 Coordinator, theHealthierHereCommunity Hub’s case management platform),authorize and enroll clients in services, referclients to direct service providers, and manage the process ofcreating, printing and mailingapprovaland service denial letters.The HRSN Eligibility &Referral Specialist will also have the responsibility of managing thecompetitivebidding process to sourceappropriate vendorsforHome Accessibility Modifications and Remediations.TheHRSNEligibility andReferral Specialistwill need to develop strong familiarity withtheHealthierHereCommunityHuband HRSN programtechnology,functions, workflows, andcommunity resourcesto be able to optimally supportthe program and its partners.

The ideal candidate brings experience in health or community services and has providedcarecoordination, health navigation, and client assessments directly to underserved individuals, families, and communities. They understand home modifications and adaptations that help seniors and people with disabilities remainsafelyin their homes and build authentic, respectful relationships with people from diverse backgrounds.

QualificationsKey Responsibilities

• Review and confirmMedicaid statusand other criteriatodetermineclient eligibility forMedicaid waiver funded Health Related Social Needs (HRSN) servicesintheProviderOnesystem.
• Enter eligibility data into the Connect2 Coordinator(C2C)system.
• Respond to referrals for HRSN services from Community Hub CBWs and contracted service providers.
• Pre-screen potential HRSNrecipients using the HRSN decision tool todeterminelikely eligibility.
• Complete HRSN assessments withlikely eligibleclients over the phonewhiledocumenting intheGetCaresystem.
• Enroll eligibleclients in HRSN services andcompletesservice authorizationsin theGetCaresystem.
• Manage thecompetitivebidding process for clients whoare authorized toreceive a Home Accessibility Modification or Home Remediation service, working with contractors,partners,andADS staff.
• Participate as a member ofthe Home Access Review committee.
• Draft, print,and mail approval,denialand termination notices as applicableforall clients screened in theGetCaresystem.
• Meet as needed with HRSN direct service providers and City of Seattle Aging and Disability Services.
• Supportthe team withmonthlyProviderOnerechecks of allHealthierHereCommunity Hub and HRSN clients todetermineongoing eligibility for HRSN services.

Qualifications

• At least2years ofcombinededucationand/or professionalexperience in health, human services, socialservicesor related fields.
• At least 2years ofexperience providingcarecoordination and/or health navigation services directly to underserved patients/clients, families, and community members, including performing assessments.
• Knowledge ofhome modifications and adaptationsto supportseniors and people with disabilities to age at home safely.
• Strong customer service skills and a demonstrated ability to connect and engage authentically and respectfully with community members with diverse backgrounds.
• Excellent written and oral communication skills, strongattentiontodetail,organization, time management,follow-through,administration,andcoordination
• Ability to work with creativity and flexibility, both independently andinterpersonally
• Demonstratedproficiencyusing Microsoft Office, Word, Excel, SharePoint, PowerPoint, and other relevant computer programs to develop materials,monitor, and communicate progress.

Preferred Qualifications

• Experience managing construction bidding processes forseniors and people with disabilities.
• Experience working with Provider One,GetCareand/or Connect2 Coordinator.
• Spanish languageproficiency

Physical Demands

• Perform prolonged periods of sitting or standing while working on a computer orattending virtual and in-person meetings.
• Engage in repetitive motions such as typing, scrolling, and using a mouse or trackpad.
• Communicate effectively through verbal and auditory means during video calls, phone calls, and in-person interactions.
• Perform occasional physical tasks, such as setting up equipment or handling materials, which may involve lifting or moving items weighing up to 20 pounds.
• Travel to the office or other work sites as needed, which may involve driving or using public transportation.
• Maintain visual focus and acuity during extended screen time.

Work Schedule

• Schedule:Monday-Friday during operating business hours. This position is subject to the organization's required hybrid work model. Work schedules and in-office expectations will be based on the position’s assigned hybrid work tier and organizational policy.
