Job summary

Western New York Independent Living, Inc. seeks an Olmstead Housing Specialist in Buffalo, NY to help clients transition from institutions to community housing.

You will identify housing opportunities and guide participants through enrollment, inspections, and moving support while coordinating with care teams. Required are an associate or bachelor’s in human services, two years in the field, and strong written/verbal communication with solid organizational skills.

Qualifications

• Associate or bachelor’s degree in human services or related field.
• Two years of experience in a related human services field required.
• Experience in housing, Independent Living, or disability rights preferred.

Responsibilities

• Identify housing opportunities for individuals transitioning from nursing homes.
• Assist eligible participants through enrollment steps, housing applications, and moving support.
• Collaborate with discharge planners, care managers, and supports to ensure service plans.
• Document activities and track eligibility, reporting as required.
