Qualifications

• Associate degree in Human Services or related field requirement.
• Bachelor’s Degree in Human Services or related field requirement.
• Professional and life experience may be considered in lieu of academic credentials.

Responsibilities

• Manage the assignments of OHS consumers for the region to Housing Specialists.
• Ensure coordination and communication between Housing Specialists.
• Identify housing opportunities for individuals who are in need of housing to leave a nursing home.
• Assist eligible OHS participants through all steps necessary for enrollment, including housing applications and moving expenses.
