This community-based initiative serves approximately 25 low-income households and 80 unduplicated individuals annually in Worcester County, specifically targeting individuals, families, and Transition-Aged Youth (TAY, ages 18-30) facing housing instability, system transitions, and economic hardship.
The Specialist works directly on the front lines to help participants navigate the transition from homelessness, incarceration, foster care, or institutional settings into stable, long-term housing and self-sufficiency.
