Job summary

Pittsburgh Mercy is seeking a dedicated administrative and social services support professional for Medical Respite. The role includes completing intakes, data entry into HMIS within 24 hours, and ensuring compliance with policies, reporting to funders, and maintaining a safe, healthy environment.

You will conduct social services assessments, collaborate with Housing Coordinator to update individual goal plans, and assist individuals with daily living needs while pursuing housing-first support.

Qualifications

• Bachelor’s Degree in Human Services or High School Diploma with 4 years of related experience required.
• Must have exceptional computer skills using MS Office suite of products.
• Must be able to obtain ACT 33/34 and FBI clearances

About The Role

Provide direct administrative, social and programmatic services support for the individuals served within Medical Respite. Administrative skilled services include, but are not limited to, compliance with all policies and procedures of the Emergency Shelter as well as reporting matrixes for compliance with funders (which directly impacts funding); operational directives to maintain a safe and healthy environment; facilitates building security; completes intakes and assessments and data entry into the HMIS system within 24 hours; accurately completes all required documentation; and other duties as assigned. Programmatic and Social Services support include, but are not limited to, social services assessment of all new as well as current individuals served and utilizing such information to collaborate with Housing Coordinator/Advocate to complete and/or update individual goal plans; provide necessary support for access to items to maintain daily living while an individual served is engaged at Medical Respite; monitor and deploy de‑escalation techniques to maintain a safe and healthy environment for all individuals served; assists individuals served with all necessary documentation for moving towards a housing first model of operations and support; and other duties as assigned. Actively engage in a team-based model of service delivery, whereby the Housing Stabilization Coordinator will be proactive in seeking additional external and internal resources to meet the needs of the individual served.
