Job summary

Housing Forward is seeking a Transitional Assistance Coordinator to advance housing stabilization for households receiving transitional aid. You will support case managers, convene coordination meetings, and connect housing, behavioral health, employment, and community services to remove barriers and reach stable, permanent housing outcomes.

Qualifications

• 2+ years of experience in human services or related field.
• Reliable transportation and valid driver’s license.
• Flexibility to work at different sites.
• Ability to lift up to 25 lbs.

Responsibilities

• Support case managers in advancing housing stabilization plans.
• Lead housing stabilization meetings with partners.
• Coordinate services across housing, behavioral health, and employment.
• Prepare regular progress updates and track actions.
• Maintain records of decisions and follow-ups.
