Job summary

Unison Health in Ohio seeks a dedicated clinician to provide direct therapeutic services to clients with mental illness, through group and individual therapy, and through diagnostic assessments. The role includes stability coordination for housing-first clients, benefits coordination, and in-home therapy as needed.

Requires an LSW or LPC (independent license preferred) with prior mental health experience. Driving is required and CPR/CPI training within 90 days is expected.

Qualifications

• Requires LSW or LPC license; working toward independent license preferred.
• Prior experience in mental health services.
• Ability to establish and maintain therapeutic relationships with clients.
• Ability to analyze problems, data, and draw valid conclusions.

Responsibilities

• Provides Stability Coordination to an assigned caseload in Housing First Program to support housing stability.
• Assists clients in establishing a budget and coordinates benefits (SS, Medicare, Medicaid).
• Assesses needs to enable independent living and links to services.
• Monitors client status with regular contracts and additional services as needed.
• Coordinates care with mental health providers and documents per agency policy.
• Attends required meetings, trainings and webinars related to the program.
• Arranges transportation to facilitate transition processes (SS office, social services, banks, stores).
• Completes diagnostic assessments for enrollment in Housing First or Supportive Services programs.
