Qualifications

• Bachelor’s degree in social service or related field, or equivalent experience.
• Minimum two years case management experience with unhoused populations.
• Excellent verbal and written communication skills.

Responsibilities

• Provide housing transition/navigation services for participants.
• Conduct needs assessments and create treatment plans.
• Assist participants in obtaining housing and necessary documents.
